Preparing For Your Visit

The initial consultation can be exciting and eye-opening.

Dr. Matthew McAuliffe and the team here at Arizona Performance Medicine is here and near you to help you transition to becoming the best you that you can be through bioidentical hormone replacement therapy and other treatments. Welcome to our family!

Please fill out the necessary forms prior to your scheduled appointment. You can come in early to fill them out, print them out at home and bring them with you, or fill them out online. This helps us get to know you and your needs and helps the clinic remain on time for all appointments.

What To Expect On Your First Visit:

Plan to spend 60-90 minutes at your initial consultation. At your visit, Dr. Matthew McAuliffe will review the intake forms filled out before the visit. Your complete medical history and lab test results will be reviewed, including medications and supplements you already are prescribed. A personalized treatment plan, including bioidentical hormone replacement therapy, will sometimes involve further lab testing to help us optimize your wellness plan. These lab tests will include evaluating your hormone levels to determine if you have any hormonal imbalances. We will also discuss any symptoms you are experiencing, such as brain fog, hot flashes, etc.

How to prepare:

• Fill out your new patient paperwork
• Bring a list of current medications
• Bring the bottles of all vitamins/nutritional supplements so your provider can evaluate all the ingredients they contain

Payment:

We do not accept insurance. Membership plans include a monthly fee. Additional fees may apply for certain medications and supplements supplied. Please see our Membership page for more details. Payment is due at the time of service.

Lab Testing:

We often require baseline lab values before tailoring your wellness optimization plan. It is possible your insurance will cover these tests. We can send you to the lab your insurance prefers, write a form that can be taken to the lab, or order the labs to have a mobile lab service come to your home or place of work if most convenient.

Not all insurance covers all blood tests. We highly encourage you to check with your insurance before assuming your insurance will cover it. If your insurance does not cover the labs, the cost can be quite high if drawn at a commercial lab service such as Labcorp or Quest. There are affordable alternatives to having lab work done that we can help provide if preferred. Please contact your Patient Liaison to find out more.

It is our goal to protect you from expensive lab bills. We can help before you get the labs. Unfortunately, once you get labs done, if insurance doesn’t cover it, there is not much we can do. Please be prepared.

Cancellation Policy:

If you cannot keep your appointment, we request a 24-hour notice to serve patients waiting for appointments. We understand unforeseen circumstances arise, but you may be charged a fee for missed appointments at the practice's discretion.
