How the Membership Plan works

Each of these membership options offers you a variety of services in addition to prescriptions and appointments. Your doctor will consistently monitor your progress in person and through phone calls. You can cancel your membership at any time. Simply contact our office and we can answer your questions.

Classic Membership for Women

  • Initial Consultation
  • 2 follow up visits in first year, 3 thereafter
  • follow up lab work
  • Prescriptions for Testosterone, Progesterone and Estrogen with shipping fees included
  • Annual fee of $3156.00 divided into 12 monthly payments of $263.00

Elite Membership for Women

  • Initial Consultation
  • 2 follow up visits in first year, 3 thereafter
  • follow up lab work
  • Prescriptions for Testosterone, Progesterone and Estrogen with shipping fees included
  • Concierge appointments as needed
  • Annual fee of $3506.00 divided into 12 monthly payments of $305.00

Classic Membership for Men

  • Initial Consultation

  • Prescriptions for Testosterone,  with shipping fees included
  • Annual fee of $2405.00 divided into 12 monthly payments of $200.00

Elite Membership for Men

  • Initial Consultation

  • Concierge appointments as needed
  • Annual fee of $2905.00 divided into 12 monthly payments of $242.00

How to get Started

We would love to provide you with your own unique, individualized treatment plan with one of our membership options. To get started, simply visit us or call our office and let us know which package you would prefer. Our receptionist will take your initial payments over the phone. For your convenience, we will store your card on file and bill the remainder of your payment fee in monthly installments.
